Main duties of the job

You will:

  • Undertake complex clinical triage and assessment using a range of consultation methods.
  • Take effective clinical histories, identify red flags and escalate concerns promptly.
  • Use clinical judgement and evidence-based triage tools to determine the most appropriate care pathway.
  • Initiate investigations, interpret diagnostic results and plan, manage and evaluate care where appropriate, with support from senior clinicians.
  • Make referrals for medical assessment and diagnostic procedures within agreed pathways.
  • Signpost and refer patients to appropriate health and social care services to reduce unnecessary A&E attendances and hospital admissions.
  • Support safe discharge into community settings.
  • Communicate complex and sensitive information clearly and compassionately to patients, carers, families and professionals.
  • Maintain accurate, complete and contemporaneous clinical records.
  • Contribute to safeguarding, infection prevention, clinical governance, audit and service improvement.
  • Deputise for the Band 7 Practitioner and support the day-to-day organisation of service delivery when required.

Detailed job description and main responsibilities

Key responsibilities include:

  • Delivering safe, evidence-based clinical assessment and triage.
  • Recognising deterioration, clinical risk and red-flag symptoms and taking appropriate action.
  • Balancing patient safety, clinical guidance and individual patient preferences.
  • Working collaboratively with patients and their families to promote independence and develop appropriate care plans.
  • Coordinating referrals and access to community, medical and diagnostic services.
  • Communicating effectively with internal and external stakeholders and ensuring clinical information is shared promptly and securely.
  • Managing anxious or distressed callers with compassion, active listening and clear, jargon-free communication.
  • Safeguarding children and adults at risk and reporting concerns immediately.
  • Maintaining confidentiality and complying with information governance and data protection requirements.
  • Recording clinical assessments, decisions and care accurately and entering activity data within agreed timescales.
  • Undertaking direct patient care within the postholder’s scope of practice.
  • Rotating into clinical teams periodically to maintain current knowledge of community services and face-to-face clinical practice.
  • Participating in clinical supervision, reflective practice, audits, surveys, quality initiatives and the development of clinical guidelines.
  • Supporting improvements to clinical practice, policies and service processes.
  • Providing cover for colleagues and deputising for the Band 7 Practitioner when required.
  • Supporting colleagues through professional advice, coaching, mentoring and appropriate training.
  • Reporting incidents and concerns relating to patient care, health and safety or service delivery.
  • Completing mandatory training and maintaining professional registration and competence.
  • Using equipment and Trust resources safely, responsibly and sustainably.
  • Working flexibly across Trust sites, weekends and bank holidays as required by the service rota.
