Care Support Worker

<p><strong>All about the Role </strong></p> <p><strong>Peterborough City Council's Reablement Team is seeking a motivated and caring individual to join the team as a Care Support Worker.</strong></p> <p>In this role, you will help maximise individuals' wellbeing and promote their independence by supporting and monitoring clients throughout their reablement journey. </p> <p>You will work with clients to support them through their reablement time. You will also work with colleagues to support individuals to achieve their goals and complete our Reablement support workers assessments.</p> <p>As a Care Support Worker, you will collaborate with colleagues and partner agencies to deliver effective reablement support. You will also undertake Adult Social Care assessments to identify longer-term care and support needs, and carry out six-week reviews to evaluate progress and ensure appropriate ongoing support is in place.</p> This role involves working Monday - Friday 9-5, with no weekend work!!
