Role Overview

The Clinical Lead in Community Care provides strategic and clinical leadership across community-based services, ensuring safe, inclusive, and person-centred care. This role is pivotal in shaping responsive care pathways, empowering frontline teams, and embedding service user voice in every aspect of delivery. Ideal applicants will be covering the East of England area, covering complex care packages across Norfolk, Suffolk and Essex.

This is a pivotal role within Clover Care Group. You will combine clinical expertise with strong leadership, supporting our nursing and care teams to deliver safe, effective and genuinely person-centred care.

Working across a portfolio of complex care packages, you will help ensure that clinical standards remain consistently high, risks are effectively managed, and our teams have the knowledge, confidence and support they need to deliver exceptional care.

You will also play an important role in the continued development of our services, working collaboratively with the people we support, families, commissioners, multidisciplinary teams and other key stakeholders

Clinical Governance & Quality Assurance

  • Lead clinical oversight of community care services, ensuring compliance with CQC and safeguarding standards.
  • Drive continuous improvement through audits, reflective practice, and co-produced quality frameworks.
  • Embed trauma-informed, inclusive approaches that reflect the lived experiences of service users.

Team Leadership & Development

  • Supervise and mentor nurses and support workers
  • Facilitate training, reflective supervision, and professional development aligned with care-led values.
  • Foster psychologically safe environments that support staff wellbeing and collaborative problem-solving.

Service Design & Transformation

  • Co-design care pathways with service users, families, and stakeholders to address social isolation and promote inclusion.
  • Analyse data and feedback to inform strategic planning and resource allocation.

Stakeholder Engagement

  • Build partnerships with local authorities, voluntary sector organisations, and advocacy groups.
  • Represent the service in strategic forums, promoting care-led leadership and service user voice.
  • Champion inclusive communication and accessible service information.

Complex Care Management

Provide recent clinical oversight and guidance for individuals with complex and changing healthcare needs, which may include:

  • Airway management.
  • Enteral feeding and associated care.
  • Suprapubic catheter care.
  • Spinal injury care.
  • Neurological and degenerative conditions.
  • Other complex health needs requiring nurse-led or nurse-delegated interventions.

You will ensure that care plans, risk assessments, clinical protocols and competencies are appropriate, current and tailored to the individual.

About You

You will be an experienced Registered Nurse with a strong background in community-based complex care and the confidence to lead clinical practice across multiple care packages.

You will combine excellent clinical judgement with a compassionate, collaborative leadership style and a commitment to delivering care that puts the individual first.

Essential Qualifications & Experience

  • Registered Nurse with current and unrestricted professional registration.
  • Significant experience within complex community care which should include writing care plans.
  • Demonstrable clinical leadership experience.
  • Experience supporting individuals with complex health needs in community or home-based settings.
  • Strong knowledge of safeguarding, clinical governance, risk management, person-centred care inclusion, and co-production principles
  • Experience leading, supervising or supporting geographically dispersed clinical and care teams.
  • Experience of nurse-delegated clinical tasks and competency assessment.
  • Strong understanding of quality assurance and continuous improvement.
  • Full UK driving licence and willingness to travel across Norfolk and Suffolk
