We are seeking a dedicated and compassionate Family Early Help Practitioner to join our team under the reference LBS-009. This role is ideal for a Social Care Qualified professional who is passionate about making a positive impact in the lives of families and children. As a Family Early Help Practitioner, you will work closely with families to assess their needs, provide timely support, and coordinate services that promote well-being and resilience.

Your expertise will help prevent issues from escalating and enable families to thrive in their communities. The position requires a committed individual who can engage empathetically with diverse families, deliver tailored interventions, and work collaboratively with a network of social care professionals and agencies. Responsibilities

Conduct comprehensive assessments to identify family needs and risks.

Develop and implement tailored early help plans in collaboration with families.

Provide practical support and guidance to families to enhance their coping strategies and resilience.

Coordinate with multi-agency partners to ensure holistic service delivery.

Monitor and review progress regularly, adjusting support plans as necessary.

Maintain accurate records and documentation in accordance with statutory requirements.

Advocate for families to access appropriate community resources and services.

Participate in team meetings, case conferences, and professional development activities.

Hours Per Week: 36

Start Time: 09:00

End Time: 17:00

Pay Per Hour: £22.86
