Registered practitioners oversee care and treatment, promoting social inclusion and recovery in line with the “Capabilities for Inclusive Practice” framework. Assist clients with complex needs to aid rehabilitation and support community-based care, working alongside care coordinators. Facilitate access to community activities tailored to clients\' needs. Principal responsibilities Establish therapeutic interactions with clients and carers under supervision of senior practitioners. Engage service users in care plans and ensure access to appropriate services based on their needs. Promote social inclusion, recovery, independence, and empowerment within safe practice standards. Participate in group projects and deliver designated activities. Plan and coordinate daily tasks effectively. Report changes in clients’ mental health or social situations, identify risks, and take appropriate action. Assist with medication administration, physical examinations, blood work, and outpatient appointments. Maintain accurate service user records in accordance with Trust policies. Identify barriers to social inclusion and work to address them. Collaborate with local organisations to support clients. Respect cultural differences and respond tactfully. Participate in staff appraisals and training sessions. Adhere to Trust rules, regulations, and policies. Experience NVQ Level 3 or equivalent in health or social care.
