Job overview

A full‑time Rehabilitation Nurse responsible for managing a caseload of patients in the Rehab unit, delivering person‑centred, evidence‑based care and working collaboratively with multidisciplinary teams. The role supports the transition of patients from hospital to community settings and may act as senior Staff Nurse when required.

Main duties of the job

  • Provide high‑quality nursing and rehabilitation care to a defined caseload of patients, carers, and families.
  • Liaise with community teams, GP surgeries, statutory and voluntary agencies to facilitate safe discharge.
  • Conduct holistic patient assessments and document findings in accordance with Bromley Healthcare policies; includes falls, continence, wound care and outcome measurements.
  • Participate in multidisciplinary activities such as case conferences, joint assessments, goal setting, team meetings and discharge planning.
  • Plan, implement and evaluate evidence‑based care programmes.
  • Ensure accurate and timely recording of all electronic and manual data.
  • Maintain community nursing standards and engage in education, peer audit and improvement activities.
  • Collaborate on health promotion strategies and activities.

Qualifications and training

  • First‑degree level education or equivalent ENB.

Professional registration

  • Level 1 registered Nurse with current NMC registration.

Specific skills

  • Ability to undertake holistic patient assessments and develop appropriate care plans.
  • Capacity to make autonomous decisions.
  • Experience with catheterisation.
  • Knowledge of wound management.
  • Evidence of directing and coordinating care plans.

Experience

  • Post‑registration experience delivering nursing care in rehabilitation or community settings.
